为适应医疗器械监督管理工作的需要，国家食品监管局组织有关专家对胶原蛋白软骨载体等43种产品进行了分类 界定，并于日前公布了这些产品的分类界定，其中14种产品划归Ⅲ类医疗器械，21种产品划归Ⅱ类医疗器械， 2种产品划归 I类医疗器械，6种产品不作为医疗器械管理。　　作为Ⅲ类医疗器械管理的产品有：胶原蛋白软骨载体、男性治 疗仪、阴茎增大器、对阴茎短小者有较好的治疗和调节作用,早泄控制器、半导体激光治疗机、激光熔脂仪(该产品如为强激光，作为Ⅲ类医疗器械管理；如为弱激光，作为Ⅱ类医 疗器械管理)、电波治疗仪、植入物包、重组人脱细胞真皮基质、脱细胞角膜基质、种植体密封胶、急救用气动呼 吸系统、一次性血液成分分离导管、细胞培养袋。　　作为Ⅱ类医疗器械管理的产品包括：湿性伤口敷料(不包括 内填药物、生物制剂或由动物材料制成的闭塞性伤口敷料)、冷喷剂、壳聚糖创伤敷料、热磁理疗仪(该产品如有 明确的适应症，作为Ⅱ类医疗器械管理；否则不作为医疗器械管理)、熏蒸治疗器(不含药)、根管润滑剂、痤疮 热疗仪、心电记录检测仪、深部静脉血栓防治系统、超声造影分析软件、真耳测试仪、振动叩击机、头皮夹子系统 、牙科用稳固度检测系统、子宫托、急救包、乳头吸牵器(该产品如用于治疗乳头内陷等疾病，作为Ⅱ类医疗器械 管理；否则不作为医疗器械管理)、洗手衣(该产品若为无菌产品，作为Ⅱ类医疗器械管理；否则不作为医疗器械 管理)、手工血细胞计数器、刀头清洁片和电极清洁片(若本产品用于外科手术中，应为无菌产品，作为Ⅱ类医疗 器械管理；否则作为I类医疗器械管理)、超声探头穿刺架(不含穿刺针，该产品若为无菌医疗器械，作为Ⅱ类医 疗器械管理；否则作为I类医疗器械管理)。阴茎增大术分为阴茎延长和阴茎增粗两种，手术目的是使正常阴茎的 外观更长更大，还可以矫正功能不正常的阴茎。　　在阴茎延长的各种术式中有3种最常用：①通过吸除耻骨上区的皮下脂肪使被包埋的阴茎凸现出来，常用于肥胖患者；②通过 分离切断阴茎悬韧带使阴茎海绵体脱离耻骨联合，达到阴茎外观延长的效果，术后常需行阴茎牵引以防回缩；③通 过将耻骨下方皮瓣向阴茎迁移而达到阴茎外观的延长。　　阴茎延长手术的成功率和并发症发生率还缺乏可信数据 。一些报道认为，松解阴茎悬韧带会减小阴茎勃起时上抬角度，而且术后阴茎可能会再次缩短。术中分离松解悬韧 带时会产生一个死腔，如果在死腔内填塞硅胶物质会防止阴茎再次回缩。皮瓣迁移可导致严重的阴茎外观畸形。如 采用大块皮瓣迁移还可导致皮瓣血运障碍、伤口延迟愈合、大块瘢痕形成等。　　阴茎增粗手术引起的争议更大，目前尚无明确的手术适应证。Wessels等认为，这种手术缺乏美学依据，无法制定相 关手术操作规范。阴茎增粗是借助皮下异种组织植入（游离脂肪或游离皮瓣等）或静脉补片增大海绵体直径等方法 来实现。很多医生采用自体脂肪皮下注入使阴茎增粗，但由于术后脂肪再吸收和局部脂肪球形成，使阴茎外形凹凸 不平，手术1年后注入的脂肪组织残存量尚不足30%，效果不理想。　　Li等进行了一项为期8年的研究，使 人们对阴茎延长手术的客观效果和患者满意度有了更新认识。研究显示，对于阴茎大小正常而要求施行增大术的男 性，应劝导他们放弃手术，改为心理治疗。希望阴茎增大的男性，阴茎往往是正常的，心理治疗可明显改善性生活 质量，无需手术治疗。　　从外科学角度来讲，阴茎增大手术缺乏科学、系统的研究。探寻非手术治疗方法是今后 的研究方向。需开发新的仪器设备，采取随机研究的方法来评估患者主观感受，重新制定阴茎大小的正常值范围。
